This patient came to Thomas Lamperti, MD to have a rhinoplasty to reduce the size of her nose. She wanted to make her tip less bulbous and to make her bridge smoother. Dr. Lamperti noted that the patient had a very boxy tip with a significant hanging columella (creating excessive nostril show) and bridge bump. Dr. Lamperti performed an open septorhinoplasty to address the patient's concerns and make the desired changes to her nose. In the 13 month after photos you can see that her bridge is more smooth and that her columella show is much improved. Dr. Lamperti also refined the patient's tip cartilages in addition to bringing them closer together with stitches in order to transform her tip to a triangle shape from a trapezoidal shape (best seen on the base view photos). He also placed a plumping graft in the infratip lobule area (the region below tip tip defining points) to act as a filler to prevent a clefted (depressed) appearance. You can also see how the patient's nostrils were initially very asymmetric on base view with a twisted tip appearance. This was due mainly from the fact that the right columellar cartilage was longer than the left. To improve the symmetry Dr. Lamperti overlapped the longer right cartilage on itself to shorten it and to allow the columella to rest more in the midline.
