Cleft Rhinoplasty Associated with Unilateral Cleft Lip

This patient has a history of prior cleft lip for which he had surgical repair as a young child. He never had nose surgery and you can see in his before photos how he has a typical external crookedness and nasal asymmetry related to his cleft lip. The patient also suffered from severe nasal obstruction due to this along with a severely dislocated nasal septum. The patient's tip was significantly underprojected and droopy and he also had a bridge bump he was interested in smoothing. Dr. Lamperti performed an open septorhinoplasty to treat the patient's concerns. Dr. Lamperti reconstructed the patient's septum which allowed him to place it in a more midline position. This opened up the patient's nasal airway which greatly improved the patient's nasal breathing. Dr. Lamperti also smoothed the patient's bridge hump while at the same time lifting it somewhat and adding tip projection. Controlled bone fractures were used to make the nasal bones straighter and cartilage plumping grafts were placed along the patient's left nasal base to create more fullness in the area and to counteract skeletal deficiency related to the patient's prior cleft lip. Asymmetrically place clocking sutures were used to further straighten the lower two-thirds of the patient's nose. In the 11 month after photos you can see how the patient has a much less crooked nose that also now has more natural appearing tip shape and position. His bridge has also been smoothed while still maintaining a masculine appearance.
