One of the most difficult maneuvers in rhinoplasty is the correction of a foreshortened, over-rotated (upturned) nose. Such nasal foreshortening can occur from birth or as an unwanted side effect of prior rhinoplasty surgery. I recently discussed the problem of the pixie nose in a real self post here.
Congenitally Short Nose
This patient had a congenitally short, upturned nose. Additionally, she has a markedly deviation nose as seen on frontal view.
So what can be done about a nose in this situation?
Lengthening a Short Nose
Using structural grafting techniques using the patient's septal cartilage the patient's nose was able to be both straightened while at the same time reducing the excessive rotation. The key to improving the short nose is having adequate support to maintain the nasal tip in its new counter-rotated position. The patient's excessive columellar show as seen on profile view was also improved.
