Case Study #1 | Bulbous Tip Rhinoplasty

As I discussed in my last blog post about fixing a bulbous nasal tip there are many approaches to improving nasal tip aesthetics.

I'd like to walk you through an example case which involved treating a patient's bulbous tip. This patient came to me looking to improve the overall size of her nose and decrease the fullness of her tip. In her preoperative photos one can appreciate the patient's broad, convex tip cartilages.



To improve the patient's tip dynamics I utilized a lateral crural overlay technique to shorten and flatten the length and convexity of her tip cartilages. In addition I trimmed a conservative amount of lower lateral cartilage to make it less broad.





After incising the cartilage and overlapping the ends, the cartilage is then sutured to itself creating a shorter, less curved strut.



The end result can be seen in these photos taken 13 months after surgery. A more refined, yet natural nasal tip can be seen. Additional improvements include reducing the overall projection of the tip, increasing tip rotation slightly, taking down the bump on the bridge and repairing the pinching in the middle third of the nose.
