Last time we reviewed what went into one of my patient's blepharoplasty surgery. Now let's see how I combined a septo-rhinoplasty with her procedure to improve her nasal appearance and function.
Nasal hump and tip over-projection to create a smaller nose
This profile photo provides a great view of the patient's over-projected tip and dorsal bump. Her nasal bump takes up almost all of her bridge except for the very upper portion and is composed of both bone and cartilage.
After trimming the excess cartilage and bone you can see how a straighter profile was created. Tip deprojection was then carried out by moving and securing the tip cartilages in a more setback position further reducing the overall size of the nose.
Bulbous tip refinement
At the same time we wanted to treat the patient's overall tip fullness that was caused by the convexity of her overlying lower lateral cartilages.
Using lower lateral crural strut grafts that were placed during the patient's rhinoplasty I was able to create a straighter orientation to these cartilages which then led to a more refined tip.
This frontal photos shows how these maneuvers allowed me to narrow the tip while at the same time creating a less rounded tip.
